Remote Diagnostic Assessment Terms and Conditions

REMOTE DIAGNOSTIC ASSESSMENT TERMS AND CONDITIONS

1. DEFINITIONS

1.1         ‘Practitioner Psychologist’: A Health and Care Professionals Council (HCPC) registered educational or clinical psychologist.

1.2         The ‘Client’: You; either the parent/carer of an Assessee or an Assessee over 18 years of age.

1.3         The ‘Assessee’: The individual who is to be assessed.

1.4         The ‘Assessment’: The SASC-compliant service provided by CF Psychology Group Ltd comprising a cognitive psychometric tests and academic achievement tests (testing), a feedback and an assessment report.

1.5         The ‘Booking’: The agreement for an Assessment of the Assessee at an agreed time and date.

1.6         The ‘Assessment Date’: The date the Assessment is booked to take place.

 

2. FORMATION OF AGREEMENT

2.1         A Booking is deemed to be made you have paid the fee for the assessment, selected a date and time and this has been reviewed and approved by CF Psychology Group Limited. By making a Booking you accept these Terms and Conditions for the Assessment. CF Psychology Group Limited reserves the right to refuse to provide a service to an Assessee for reasonable reasons including if the Assessee is not an undergraduate or postgraduate student at a UK university or college and if we refuse to provide a service we will provide a reason for that refusal.

2.2         The maximum time allocated for conducting the testing and feedback meeting is two hours; one hour and forty-five minutes for the testing and 15 minutes of feedback. The Practitioner Psychologist is not required under any circumstances to go beyond the allocated time for the Assessment and may terminate the Assessment at the end of the allocated time.

2.3         The testing will be carried out by a Practitioner Psychologist using video conference.

2.4         CF Psychology Group Ltd. requires the Client to be truthful and work in the best interests of the agreement between CF Psychology Group Ltd. and the Client. The Client agrees to provide CF Psychology Group Ltd. with all pre-Assessment written work such as questionnaires or copies of previous assessments. The Client agrees to complete all the required tasking as issued by the CF Psychology Group Ltd. within the timeframe it was requested. The Patient also agrees to provide all written pre-Assessment work to reach the CF Psychology Group Ltd. by email within 48 hours of making the booking. Any documents provided after this time may not be considered. If information provided within 48 hours of the Assessment leads us to the opinion that the Assessment is not in the best interests of the Assessee, we reserve the right to cancel your Booking and a 100% cancellation fee will be incurred.

2.5         Lateness: You agree that arriving on time for the Assessment and to the Assessment location is fully your responsibility. The Assessment will be terminated if you arrive over 20 minutes late and no refund will be given in any circumstances even if the Client wishes to cancel the session due to emergency or unforeseen circumstances out of their immediate control.

 

3. FEES

3.1         The Client (you) agrees to be fully responsible for all fees relating to the services delivered to yourself or the Assessee.

Pricing and payment

3.2         The cost of the Assessment is £375.00 and is payable when your booking is made.

Additional Practitioner Psychologist time

3.3         If you require additional time from a Practitioner Psychologist following the Assessment, for example additional telephone calls, letters, this is charged at a rate of £50 per hour. Payment is required in full prior to the commencement of this work. Whilst every effort will be made to arrange this work with the same Practitioner Psychologist who conducted the Assessment, this will be at the discretion of CF Psychology Group Ltd. and if it is not possible then it will be carried out by the Principal Educational Psychologist.

 

4. RESCHEDULING OR CANCELLING

4.1         We hope it is appreciated that once an appointment is made, we rely on our Clients to do their best to keep to it. For every appointment made, our service incurs costs for Practitioner Psychologist preparation and practice time, administration, telephone calls and other variable costs. More importantly, if appointments are missed or cancelled at short notice, we are unable to offer those appointments to other Clients because of the preparation time required before an Assessment, for example gathering completed school questionnaires. However, we do understand that occasionally, Clients will need to postpone or cancel their Assessment.

If you need to cancel or postpone your Assessment, please let us know as early as possible. This may enable us to offer the appointment to another Client.

Rescheduling

4.2         If you need to reschedule your appointment, for example due to ill-health, we ask that you inform us at the earliest possible opportunity. Your appointment will be rescheduled to a date and time that is convenient for you but no sooner than the next available date and time we have in our schedule. There is no cost incurred if you need to reschedule an appointment with more than 48 hours notice. If you need to reschedule with less than 48 hours notice then our cancellation policy below will apply.

4.3         There may be times when a Practitioner Psychologist is unable to provide the service at the agreed time and date, for example if they are unwell. We will always endeavour to allocate an alternative Practitioner Psychologist from our team but if this is not possible, we reserve the right to reschedule your appointment. In such cases, we will inform you at the earliest opportunity and your appointment will be rescheduled to a date and time that is convenient for you but no sooner than the next available date and time we have in our schedule.

 

Cancelling

4.4         You may cancel your Booking up to 48 hours prior to the Assessment Date and time and receive a full refund. If you cancel your Booking less than 48 hours before the Assessment date and time, or do not attend your appointment, or do not ensure the correct remote assessment requirements (as per section 5) are met, a £75 cancellation fee will be incurred as we will be unable to re-use the time to support other Clients.

 

5. REMOTE ASSESSMENT REQUIREMENTS.

5.1         You must join the Assessment video call using either a computer (PC or Mac) or tablet, with a working microphone and        camera. You CANNOT take part using a smartphone.

5.2         A suitable internet connection is required.

5.3         You must be in a quiet, private room.

5.4         You must have a pen and lined paper.

5.5         Audio or video recording of the Assessment, by any means, is strictly prohibited.

5.6         Glasses- If you wear correctional glasses please ensure these are brought with you to the Assessment.

5.7         Medication- If you take medication, please ensure this is taken as normal prior to the Assessment.
